• Invitation of an established type issued by the Ministry of Education of Ukraine .
  

• Recent bank Statement of parent

• Appointment Letter of Parents.

•  General Medical Certificate legalized by the Ministry of Health and the MFA of  YOUR COUNTRY issued not later than two months before entry to Ukraine and translated into Ukrainian.  

•    Medical Certificate confirming absence of HIV / AIDS legalized by the Ministry of Health and the MFA of YOUR COUNTRY issued not later than two months before entry to Ukraine and translated to Ukrainian.

• Transcript of school leaving Certificate of general education schools and college, university diploma etc to be legalized in the Ministry of Foreign Affairs(MFA) of YOUR COUNTRY  and translated into Ukrainian Language.

• Certificate of the Ministry of Education of YOUR COUNTRY, which confirms accreditation of General educational schools and college, university etc and right of a graduate to continue study in the Higher school, to be legalized in the Ministry of Foreign Affairs of YOUR COUNTRY and Translated into Ukrainian Language.

• Copy of the birth Certificate to be legalized in the Ministry of Foreign Affairs of YOUR COUNTRY and to be translated into Ukrainian Language.

• Insurance Policy concerning urgent medical assistance legalized by the MFA of YOUR COUNTRY.

 Procedures For Foreign Applicants To Ukraine.

1. Foreigners - candidates for obtaining education in ONMU- shall submit the following documents:
a) the questionnaire;
b) a copy of the document of education with the list of studied courses (subjects) and received grades (points);
c) the document evidencing the absence of HIV-infection, unless otherwise provided for by international agreements of Ukraine;
d) the medical certificate of the state of health certified by an official health care authority of a country, where a foreigner has come from, and issued at least two months prior the entrance into Ukraine for education;
e) the insurance policy for emergency medical care (except for foreigners arrived from countries, with which agreements on free provision of emergency medical care are concluded);
f) a copy of the certificate of birth;
g) 10 photographs having the size of 6 x 4 sm;
h) return ticket with open date.
2. Persons, who enter the postgraduate studies or studies for obtaining doctor's degree, shall additionally submit a list of the published scientific-research works with the specification of a place of publishing, and such works shall be certified by a head of educational institution, organization at the place of work. Postgraduates, who have no published works, shall submit research papers (reports) in the are of the scientific profession selected by them.
When admitting to the studies for taking doctor's degree, the following documents shall submitted additionally:
a)comprehensive plan of a dissertation work for taking the scientific degree of a doctor of science;
b)copy of the diploma on awarding the scientific degree of a candidate of science.
3. Documents set forth in Articles 1 and 2 shall be in the Ukrainian, Russian or English languages, and documents set forth in paragraphs "b", "c", "d" and "f" of Article 1 and paragraph "b" of Article 2 shall, in addition, be certified pursuant to the legislation of a country of their origin and legalized pursuant to the established order, unless otherwise provided for by international agreements of Ukraine.
